If your family is dealing with cancer, you can request a comfort box here.
There is no cost, and you don’t need to figure out exactly what to say. Give us the basic information below and we’ll reach out to make sure we have everything right before your box is put together.
You can request a box for your own family or for someone you care about.
A little about your family
The form asks who lives in the home, their ages and a few things they enjoy.
Be as specific as you’d like.
Favorite teams, hobbies, characters, music, animals, games, movies or just something they’re really into right now — all of it is helpful.
Please include everyone living in the household so we know exactly who we’re sending to.
Who can request a box?
We accept requests from families anywhere in the United States, regardless of cancer type or stage.
We especially hope to reach families who are newly or recently diagnosed, when so much is changing all at once. But there isn’t a deadline for asking for support.
If cancer is part of your family’s life right now, you are welcome to submit a request.
We provide one comfort box per household.
What happens after you submit?
We’ll contact you. Someone from the Everts Foundation will reach out within one business day to confirm the information you submitted and ask anything else we need to know.
We’ll put your box together. Once we have the details we need, we’ll begin preparing it for your family.
We’ll send it out. Most boxes ship within 5–10 business days.
We’ll send you tracking. As soon as your box ships, we’ll email you the tracking information so you know it’s on the way.
If time is especially important
If someone in the family is receiving hospice or palliative care, please tell us on the form.
We’ll make your request a priority and do everything we can to have the box on its way within one or two days.
